2010 Ground Transportation Permit Renewal

Attention Ground Transportation Providers:

It is time to submit your company’s application for renewal of your 2010 ground transportation permit. All applications must be received by December 31, 2009. A checklist of all forms and documentation that should be submitted in order to complete the processing of your company’s application has been provided. Please ensure that all required information/documentation is included to prevent processing delays.

Checklist for Permit Renewal:
  • Renewal Application Form, completed and signed.
  • Permit Document (cover and signature pages completed and signed)
  • GPSC Class ‘B’ Certificate (Limousine or Passenger Carrier, as appropriate)
  • Department of Revenue Intrastate Registration Permit (IE).
  • Department of Revenue Form ‘G’ Vehicle Identification Cabcards (intrastate), or FMCSA (interstate) Unified Carrier Registration Receipt (UCR).
  • ACORD Certificate of Liability Insurance (limits $500,000, $1,000,000, $100,000), City of Atlanta named as additional insured and State of Georgia required Automobile Liability insurance in stated amounts.
  • Insurance Verification Form, signed and notarized.
  • Non-negotiable Title and Vehicle Registrations.
  • Company Fleet Log (all details completed).
  • Company Driver List (all details completed).
  • Fee Computation Form as applicable for provider category.
  • $20.00 non-refundable Application Processing Fee payable to City of Atlanta.
