2011 Ground Transportation Permit Renewal

Attention Ground Transportation Providers:
It is time to submit your company’s application for renewal of your 2011 ground
transportation permit. All applications must be received by November 30, 2010. A
checklist of all forms and documentation that should be submitted in order to complete
the processing of your company’s application has been provided. You must ensure
that all required information/documentation is included to prevent processing delays.
